Population health is about improving the health of communities by supporting your care at every stage. That means helping you stay well, identifying issues early and making care easier to access when it’s needed.

At Hartford HealthCare, we do this by understanding our patients, partnering with our communities and using technology to support care.

Caring for communities across Connecticut

We provide care to over a million patients across 500+ locations in 185 towns and cities in Connecticut.

In 2025, Hartford HealthCare Medical Group completed 2.6 million patient visits across primary care, urgent care and specialty care.

Our Priorities

Our work centers on improving care and health outcomes.

Many of our programs follow quality standards from the Centers for Medicare & Medicaid Services (CMS) and the National Committee for Quality Assurance (NCQA).

Across Hartford HealthCare, we focus on:

  • Blood pressure control to help lower the risk of heart disease and stroke
  • Diabetes control to help prevent long-term health problems
  • Kidney health checks for patients with diabetes to help protect kidney function
  • Breast and colon cancer screenings, because finding cancer early can make treatment more successful

Progress and Recognition

Supporting patients with diabetes

The Hartford HealthCare Diabetes Support Program showed meaningful improvements for patients at high risk for diabetes complications, according to a Quinnipiac University review.

In their report, reviewers noted that “the results demonstrate that a comprehensive, community-based intervention can meaningfully shift outcomes in a population at the highest risk for diabetes complications.”

The program was created through a grant from a major retailer. It was designed to support patients with diabetes who face higher social risk and explore how care that includes social needs can improve health outcomes.

Hartford HealthCare named 2026 Acclaim Award finalist

Hartford HealthCare was named a 2026 Acclaim Award finalist by the American Medical Group Association (AMGA).

The award recognizes healthcare organizations for high-quality, patient-centered care that supports the Quadruple Aim. This includes improving patient experience, supporting better health across communities, reducing healthcare costs and promoting workplace wellness.

Hartford HealthCare aims to present at the AMGA conference this spring on Balanced Scorecard initiatives focused on improving blood pressure control for patients with hypertension and A1c control among patients with diabetes.

Feedback from the AMGA application noted Hartford HealthCare’s strong alignment with the Quadruple Aim, positive outcomes and commitment to addressing social determinants of health.
